What is SS-31? Research, safety, and status
SS-31 (Elamipretide) is a first-in-class mitochondria-targeting peptide built to address the root cause of diseases driven by mitochondrial dysfunction, rather than their downstream symptoms. It is a four-amino-acid peptide engineered to selectively accumulate inside mitochondria, where it stabilizes the inner-membrane phospholipid cardiolipin and, with it, the machinery that generates cellular energy.
It stands apart from most peptides discussed in the longevity/performance space in two ways: it acts at the level of cellular bioenergetics (not growth-factor or hormonal pathways), and it has a real clinical-trial record, culminating in FDA accelerated approval in September 2025 (as FORZINITY) for Barth syndrome, the first approved therapy for any mitochondrial disease.

Study highlights
- SS-31 selectively binds cardiolipin on the inner mitochondrial membrane, stabilizing cristae architecture and electron transport chain supercomplexes; PNAS proteomics identified 12 direct SS-31 interactors, all cardiolipin-binding proteins involved in OXPHOS and 2-oxoglutarate metabolism.
- Ex vivo treatment of explanted failing human hearts acutely improved mitochondrial respiration and supercomplex function, demonstrating the mechanism is independent of long-term cardiolipin remodeling (JACC: Basic to Translational Science, 2019).
- Phase 2a trial in 14 patients with atherosclerotic renal artery stenosis showed that adjunctive elamipretide during stent revascularization reduced renal tissue hypoxia by 6% vs. +47% in placebo and increased renal blood flow from 202 to 262 mL/min, supporting mitochondrial protection during ischemia-reperfusion.
- The PROGRESS-HF Phase 2 trial (71 patients, HFrEF) found elamipretide well-tolerated but did not significantly improve LVESV at 4 weeks; the TAZPOWER open-label extension (168 weeks) in Barth syndrome demonstrated significant improvements in 6-Minute Walk Test and cardiac stroke volumes, leading to FDA accelerated approval of Forzinity in September 2025.
